Appointments may be scheduled by calling our office at (318) 325-2020, Monday - Thursday from 7:30 a.m. to 5:00 p.m., and Friday 7:30 a.m. to 2:00 p.m.

Please bring insurance cards, list of medicines, and your most recent pair of eyeglasses or contact lenses.

Eye examination equipment. If this is your first visit to our clinic, please arrive 20 minutes early to complete appropriate forms. You can save time by completing forms in advance. Click on each of the links below to download and print our new patient forms. You may fax them to us at (318) 388-0000 or simply bring them with you to your appointment.

Please allow an hour or two to complete the exam. If it is necessary to dilate your eyes, your vision may be blurred and sensitive to light. We encourage you to bring sunglasses and to make arrangements for someone to drive you home. We do not recommend that you drive after your eyes have been dilated, patched, or if an ointment has been applied to them.

If you are unable to keep an appointment, please call our office as soon as possible to reschedule.

The Eye Clinic of Monroe accepts many health insurances including Medicare. Please call our office to inquire if we are participating providers for your specific plan. Our friendly billing staff will be happy to address your questions.
